Our biggest compliment is a friend or family referral. We want you to share your experiences with others and be rewarded at the same time.

HOW IT WORKS

1. Refer a friend
   Have your referral fill out an “I was Referred Form”
   (download the form here or get one in our office)
2. Receive a Reward
   Once your friend completes a procedure, your reward will be added to your procedure.

RULES / RESTRICTIONS

• You will receive 10% of the cost of the referred friend’s procedure – with a limit of up to $500. For example:
  friend’s procedure cost was $1,000 – you get $100
  friend’s procedure cost was $3,000 – you get $300
  friend’s procedure cost was $5,000 – you get $500
  friend’s procedure cost was $8,000 – you get $500
• Rewards may only be redeemed on procedures
• Rewards are non-transferable and may not be redeemed for cash
• Rewards never expire!
• No limit in the number of referrals
